First off I am in Canada, so my questions are specific to our health system….
I’m hoping to get a set of dog dags made, basically a cheap medic alert system. I’m going to do my first and last name, as well as provincial health card number, and blood type.
In Nova Scotia, our health card number is called our MSI (I’m not sure of other provinces), so I feel I should list this as “MSI 000000000” that way any paramedics in NS will be able to quickly identify it is a MSI number and run it from the truck.
Next question (two parts), lets say I am out in Manitoba, or some other province. Should I list a province on a separate line (maybe “NS CAN”) to help them find my details (this may be more for the hospital, less for the medics). Also, if I list
“MSI 0000000” than “NS CAD” on the next line, will a paramedic, or even nurse at the hospital who is registering me realize this is my health card number?
Last question, I have congenital heart disease/defects. I have seen this short handed as CHD, and I would think having this as ready information for a medic would be good. My only concern here is, would a medic, or doctor, or anybody seeing CHD on a dog tag have any clue what it is?
The general format I am thinking of is:
LAST NAME FIRST NAME
MSI 0000000000
O NEG
CHD
NS CAD
