Looking for belt case for an iPhone 5 in an OtterBox

I’ve had my iPhone 5 for 6 months now and it has been inside an OtterBox case since day one. The problem is the OtterBox belt clip has broken on me twice now. The tab that snaps on to hold the phone in place has broken at the same spot both times. OtterBox sent me a replacement the first time, and I need to request another (I have to say the OtterBox online warranty system is outstanding) but now I just don’t trust it.

So I’m looking for a new belt pouch to carry my iPhone with the OtterBox case already installed. Here’s what I’d like to see:

Black real leather, not some synthetic leather knock off.
No magnetic flap closure – I have enough problems with my credit card magnetic strips.
Slip on belt loops preferred but any belt clip that won’t come off accidentally would work.
Enough room that the phone will go in/out of the pouch using only my right hand.
The ability to pop the phone in/out of the pouch with the earbuds still attached.

I cannot find such an animal. I cannot even find something close so far.

Any suggestions?
